Putting that mission into action requires working collaboratively with community partners, insurance companies, and government agencies to understand opportunities and build solutions to deliver equitable and sustainable healthcare.
Augusta Care Partners is a network of community health care providers dedicated to improving health outcomes and ensuring that Medicare members receive the right care at the right time. In partnership with Medicare (through its Accountable Care Organization program), our providers work together to create a seamless health care experience for Medicare members in our community.
The Population Health team collaborates with Augusta Medical Group and a variety of commercial insurers to deliver high value health care for community members. Using data analytics and clinical insights, we partner with our doctors to provide needed preventive care, chronic disease management, and well-coordinated services.
There are several components that contribute to a person’s overall health. By maintaining a proper balance of physical activity, proper nutrition, sleep, mental health and stress management, people can enjoy healthier lives.
The population health team strives to improve health of community members through measurement and programs tied to health-related social needs, through our focus on reducing health disparities and through our efforts to connect with underserved and disadvantaged members of our community.
Health equity exists when everyone has a fair and just opportunity to be as healthy as possible. This requires removing obstacles to health such as poverty and discrimination as well as their consequences, including powerlessness, lack of access to good jobs with fair pay, quality education and housing, safe environments, and health care (Robert Wood Johnson Foundation).
