Heart Failure Clinic
Our goal is to partner with you, to customize and optimize your treatment plan, with the goal of respectfully improving your overall quality of life. As you may already know, Heart Failure is a very serious condition. If you were recently admitted to the hospital with Heart Failure, you have a high chance of readmission. We want to partner with you to help you prevent this if at all possible. We promise to do our best in assisting you in relieving your heart Failure symptoms. We promise to be honest, to listen, and to advocate for you.
What happens at the Heart Failure Clinic?
You will receive close follow-up and monitoring
When we first begin seeing you in clinic, we may need to see you weekly. We will need to gather your medical history, perform a physical assessment, obtain your vital signs, obtain blood work, teach you and your family about heart failure management, and take time to answer any questions you may have. We will manage any heart Failure symptoms that you may be experiencing and possibly initiate treatment in clinic, with Intravenous therapy (IV therapy). You will see us less often as your symptoms stabilize and improve. We will depend on you to partner with us and to call us promptly when you have any new concerning symptoms. All care received in the clinic will be communicated to your cardiologist and to your primary care physician.
Blood Tests
We will sometimes require blood work during clinic visits. The reason for this is because some of the medications prescribed to treat your Heart Failure can affect and damage your kidneys and cause problems with your electrolytes in your blood. We want to prevent any further complications if we can, so obtaining lab work will be routine.
Medication Changes
We will make sure to initiate or increase Heart Failures specific medications that will strengthen your heart function, while removing any excess fluid with diuretics (water pills). We will educate you on these medications so that you are well informed. It will be very important for you to partner with us by monitoring and recording your daily symptoms, weight, blood pressure, and heart rate. We will review this information and adjust your medication accordingly.
