We provide community members who are dealing with diabetes and other chronic diseases with the skills and knowledge necessary to successfully manage their health and improve their quality of life.

This is a single-session program taught by a member of the Diabetes Education Staff.

A referral from your physician is required for outpatient nutrition services.
