Augusta Health’s Community Health Needs Assessment

Every three years, per federal guidelines, Augusta Health reaches out to community members and listens to the major health concerns they have. The Community Health Needs Assessment results fuel different programs and projects the hospital orchestrates with community partners. 

”Every single survey response matters into the feedback that we are given for the final report to help us understand the needs of the community,” says Krystal Moyers, Administrative Director of Community Partnership.

Folks living in the SAW area can access the survey form on the Augusta Health website, or receive a call from (540) 255-0040 which will have “AUGUSTAHLTH PRC” as the caller ID. 

The survey will be open until March 24, 2025.
