FISHERSVILLE, VIRGINIA— Augusta Health is conducting its Community Health Needs Assessment (CHNA) to enhance community health. Federal regulations mandate hospitals to perform a CHNA every three years, identifying health priorities to address the greatest needs of the community. Since 2009, Augusta Health has partnered with various organizations for a comprehensive CHNA, evaluating community health and guiding targeted initiatives to improve health outcomes in the region.
How to participate
Augusta Health launched an online survey, open until March 24, 2025, available at https://www.prcsurvey.com/begin/AugustaHealthCHNA2025. It includes key questions to ensure thoughtful feedback. Spanish-language options are accessible via a toggle at the top of the survey. The community is encouraged to participate and provide feedback regarding their personal health needs.
PRC Custom Research, Augusta Health’s survey partner, will conduct a phone survey alongside the online version. Calls will come from (540) 255-0040 and appear as “AUGUSTAHLTH PRC” on caller ID. If contacted, please take the time to respond.
Survey details and priorities
The CHNA survey will identify health challenges and opportunities to enhance community well-being. The 2022 assessment, in addition to benchmark data, and potential health challenges, identified three key focus areas for Augusta Health: Mental Health and Substance Abuse, Access to Healthcare, and Nutrition and Physical Activity.
